Where to Go for Miscarriage Bleeding: ER vs. OB-GYN vs. Urgent Care

Take a Deep Breath

First, I want to say: Not all bleeding means a miscarriage. About 25% of pregnant women experience bleeding and go on to have healthy babies.

However, you need medical answers. The biggest mistake patients make is driving to Urgent Care. Most Urgent Cares do NOT have transvaginal ultrasound machines and cannot detect a fetal heartbeat. They will simply charge you a copay and tell you to go to the ER.

So, it comes down to two choices: Your OB-GYN or the Emergency Room.

The “Pad Test” (Self-Triage)

Doctors use the amount of bleeding to decide urgency. Use this checklist.

🚨 Rush to ER If…

  • The Pad Rule: You are soaking through one maxi pad in one hour (or less).
  • Pain: Severe abdominal pain on one side (Could be Ectopic Pregnancy – Life Threatening).
  • Symptoms: You feel dizzy, faint, or have shoulder tip pain (Internal bleeding sign).
  • Clots: You are passing clots larger than a quarter.

📞 Call OB-GYN If…

  • Spotting: Light pink or brown discharge (common after sex or exams).
  • Mild Cramps: Feels like a light period.
  • Timing: It is during business hours. (They might squeeze you in for an ultrasound immediately).

Cost Reality Check

Miscarriage care is expensive, but safety comes first. Here is what to expect.

💰 Price Tag Breakdown

  • 🏥 Emergency Room: $2,000 – $4,000+
    Includes facility fee, ultrasound, and blood work (HCG levels).
  • 👩‍⚕️ OB-GYN Office: $150 – $300
    Standard office visit + ultrasound copay. Much cheaper.
  • 🩺 Urgent Care: $150 (Wasted)
    They will likely just check your vitals and transfer you to the ER.

💡 Don’t Forget the “RhoGAM” Shot

If you have a Negative Blood Type (e.g., A-, O-), this is critical.

Any bleeding during pregnancy can cause your body to build antibodies that attack future pregnancies. You need a shot called RhoGAM within 72 hours of bleeding. If you go to the ER or OB-GYN, remind them: “I have a negative blood type, do I need RhoGAM?”
